How to Request a Certificate

If you're a current Rick Allen Insurance Agency client, requesting a certificate is straightforward. Call us at 304-252-1660 during business hours or submit the form below. We'll need a few details:


  • Your name and policy information
  • The name and address of the certificate holder (who's requesting it)
  • Any specific additional insured or wording requirements from the requesting party

Most certificates can be processed and sent the same day for existing clients during business hours. If you have a deadline, let us know when you call or in the form.

What Is a Certificate of Insurance?

A certificate of insurance is a one-page document that summarizes your current coverage — policy types, carriers, limits, and effective dates. It doesn't change or extend your coverage. It simply confirms to a third party that you have insurance in place.


COIs are commonly required by:

  • General contractors or project owners before allowing subcontractors on a job site
  • Commercial landlords before a business tenant moves in
  • Lenders or banks requiring proof of business insurance
  • Government agencies or municipalities before awarding a contract
  • Event venues requiring proof of liability coverage
